Company Paid Life and Accidental Death & Dismemberment (AD&D)

Providing economic security for your family if you die, become disabled, or experience an injury or illness is a major consideration in personal financial planning. You’re provided with employee Life and Accidental Death and Dismemberment (AD&D) coverage at no cost to you.

Eligible employees automatically receive Life and AD&D coverage in the amount of one times your annual salary up to a maximum of $500,000 or $50,000, whichever is greater. Don’t forget to update your beneficiary designation if you experience a life changing event. Don’t forget to update your beneficiary designation if you experience a life change.

Voluntary Life

Voluntary Life is available to purchase additional coverage for yourself and for your spouse and children. Employees can choose amounts in $10,000 increments up to $500,000. You may purchase $5,000 increments up to 100% of your election up to $100,000 for your spouse and increments of $1,000 up to 100% of your election or $10,000 for your children. The guaranteed issue amount for employee is $200,000, $50,000 for your spouse, and $10,000 for your child (if you apply when initially eligible for coverage). For any amount over the guaranteed issue limit, Evidence of Insurability (EOI) is required. You must be enrolled in order to enroll your spouse, domestic partner or children.

Salary Continuation

Salary Continuation protects you and your family from a loss of income if you are unable to work due to an illness, injury, or accident for a short period of time. The Salary Continuation plan provides a percentage of your base pay for up to 3 months of disability and the benefit percentage amount is based on your length of service.

Long-Term Disability (LTD)

Long-Term Disability (LTD) coverage is important because anyone at any age may become injured or ill for an extended period of time. LTD protects you from loss of income in the event you are unable to work due to an illness, injury, or accident for a period of time that exceeds the short-term disability benefit.

The LTD coverage will replace 50% of your base salary to a monthly maximum of $10,000 if you are disabled for more than 90 days and are unable to work. LTD benefits start on the 91st day of being unable to work. LTD benefits are offset with other sources of income, such as Social Security and workers’ compensation.
