We’re proud to offer you the following medical plan choices designed to help you and your family get the care you need at an affordable price.

Kelsey Care Health Maintenance Organization – Houston EEs Only

The Kelsey Care Plan option provides coverage through in-network doctors (Kelsey-Seybold) from which you will select a primary care physician (PCP) who refers you to specialists if you need them. You are responsible for copays when you receive services. There is a low deductible and coinsurance for some services that are not associated with a copay.

You will not have access to out-of-network physicians, with the exception of emergency situations, if you are enrolled in this option. If you decide to seek treatment from an out-of-network doctor, you will be responsible for the full cost of treatment.

Cigna IN Open Access Plus (IN OAP)

The IN OAP option provides coverage through in-network doctors (Open Access Plus network).  You are not required to select a PCP and you are not required to get a referral to see a specialist.  This plan offers copays for office visits and prescription drugs and a deductible and coinsurance for other services.

You will not have access to out-of-network physicians, with the exception of emergency situations, if you are enrolled in this option. If you decide to seek treatment from an out-of-network doctor, you will be responsible for the full cost of treatment.

Cigna High-Deductible Health Plan (HDHP) with HSA

The HDHP HSA Plan option pairs a high deductible health plan with a health savings account (HSA). With this option, you will be responsible for most expenses (with the exception of preventive care – payable at 100%) up to a certain dollar limit, known as the deductible. After you meet the deductible, you will share a percentage of the expenses in the form of coinsurance. There is also an out-of-pocket maximum which limits the total amount you could be responsible for in a given year. If you reach the out-of-pocket maximum, the plan picks up the cost for all covered services from that point forward for the rest of the year.

When you enroll in the plan, you are eligible to open a health savings account (HSA) to set aside pre-tax dollars to use for medical, prescription drug, dental, and vision needs. You can even use it to cover the expenses of your spouse and/or dependents, even if they are not enrolled in your medical plan. HSAs have many benefits, including the following:

  • Your contribution to the HSA is a pre-taxed benefit and you earn tax-free interest on HSA balances.
  • You own your HSA, even if you change jobs or retire.
  • You don’t pay taxes when paying for qualified medical expenses.
  • Your HSA balance can be carried over year after year.
  • You can use your HSA to help add to your retirement funds.
  • After you turn age 65, you can withdraw funds from your HSA for any reason without penalty.

For 2019, the Company will contribute directly into your Health Savings Accounts $750 for an employee with individual coverage and $1,500 for an employee with family coverage for employees who elect the HDHP Plan option. Half of the Company contribution will be paid with the initial deposit and the rest will be divided evenly per paycheck for the remember of the year. (Annual amounts will be prorated for late joiners).

Additional employee contributions can be made up to the IRS maximums. In 2019, HSA holders can choose to save up to $3,500 for an individual and $7,000 for a family (including the amount of the Company contribution). HSA holders age 55 and older can save an extra $1,000.

Cigna Open Access Plan (OAP)

The OAP Plan option is a Preferred Provider Organization (PPO) plan offers the flexibility to choose an in-network or out-of-network provider each time you need care. Keep in mind, you will save money when you visit in-network providers. In-network office visits, specialist visits, and urgent care are subject to a copay with some limitations. Other services are subject to coinsurance after your deductible has been met.
